The period of performance is six months from the date of award.

Firm fixed-price payments will be made upon completion and government acceptance of each task, with a single monthly invoice submitted as appropriate.

Contractor personnel must possess qualifications such as Certified Safety Professional (CSP) or Associate Safety Professional (ASP) certification for the Project Manager/Lead, and a minimum of 3 years of occupational safety experience for Safety/Technical Staff.

Proposals must be submitted electronically by Friday, August 7, 2026, at 12:00 PM CT. Physical submission of sample materials is due by Friday, August 7, 2026, at 3:00 PM.

Payment will be made in accordance with the Illinois Local Government Prompt Payment Act. Interest will be paid if the district is late in making a payment.

Publishers warrant that goods supplied will have good title, be of merchantable quality, and be fit for the particular use intended, free from defects, and in full conformity with specifications. Warranties survive acceptance. Publisher shall promptly repair or replace goods for a period of 1 year from the date of delivery.

Award will be made based on the fees, experience, reputation and the financial stability of the publisher as well as compliance with the format, terms, and conditions of this request for proposal.

The school district may investigate the qualifications of any proposer under consideration and require confirmation of information furnished by a proposer, and require additional evidence of qualifications to perform the services described in this RFP.

Failure to comply with RFP requirements may lead to disqualification. Rejected goods shall be removed at the expense of the publisher. If publisher fails to deliver, district reserves the right to reject goods and cancel all or any part of the sale.

Sample materials (print or digital) are to be supplied at no cost to the district. Print materials are to be shipped to the purchasing department. Access to demonstration accounts for digital materials is also required.

All potential payments shall be made in accordance with the Illinois Local Government Prompt Payment Act, as amended.

Proposals will be evaluated based on organization experience and qualification (35 points), ability to meet the scope of services and program requirements (35 points), references (15 points), and cost proposal (15 points).

The treatment provider must be an Illinois licensed behavioral health provider with qualified licensed clinical staff, experience serving justice-involved populations, and the ability to collaborate with multidisciplinary teams.

Proposals may be deemed nonresponsive if they conflict with the terms and conditions, or if the respondent fails to promptly supply requested information. Disqualification may also occur upon evidence of collusion or other illegal practices.

The contract period for the base year is from 08/17/2026 to 08/16/2027. Option periods extend through 08/16/2031. Specific delivery dates for items are listed as IAW SOW followed by the contract period dates.

Invoices shall be submitted upon receipt and acceptance by the government. Payments will be made via electronic funds transfer through the System for Award Management.

Yoga instructors must have completed a 200-hour yoga teacher training course and be registered with Yoga Alliance as a Registered Yoga Teacher (RYT200). Proof of certifications, negative TB test, pre-employment drug screening, and BLS certification are required. Personnel must be proficient in English and have basic PC skills. Credentialing through VetPro is required.

Sustained allegations of patient/coworker abuse will result in immediate removal from the facility and may make the contractor ineligible for future contracts. Repeated cancellations or schedule changes by the contractor may result in contract deficiency reports.

Vendor quotes may be considered noncompliant and rejected if the contracting officer cannot verify SAM registration and certification status as a Women-Owned Small Business (WOSB).
