This is a combined synopsis/solicitation for commercial items for medical general health care services. Performance will be throughout Michigan. The period of performance is a base period of 12 months, plus CLIN **** for travel. Quotes must be submitted electronically in Microsoft 365 or PDF format and must be compatible with Microsoft 365 or PDF Adobe. The offeror must have an active registration on sam. gov at the time of award and throughout the period of performance. Technical capability and past performance will be used for evaluation, along with price. Past performance information should not be more than five years prior to the solicitation issuance date. Pricing shall be submitted in relation to the solicitation CLIN structure and include substantiating data. The offeror shall prominently list the expiration date of the pricing for a minimum of 60 days.
The submission due date for this solicitation is detailed on page 1, in block 8 of the subject standard form 1449. The offeror must ensure its quote, in its entirety, reaches the required destination before the date and time set for closing of the solicitation.
Terms of any express warranty must be included in the offer.
The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: Technical capability, Price, and Past Performance. Technical and past performance, when combined, are equally important as lowest price.
The contract will be placed only with an offeror that the contracting officer determines to be responsible, that is, those who can satisfactorily perform the necessary tasks and deliver the required services on time. Prospective offerors, in order to qualify as sources for this acquisition, must be able to demonstrate that they meet standards of responsibility set forth in FAR 9. 104. The offeror must have an active registration on sam. gov at the time of award.
When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers.
Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. An offeror failing to submit an OCI plan that it is later determined should have been submitted, could cause the quote to be rejected and no longer be considered for award.
