Request for Proposals for a Kinship Support Navigator Program 1. full kinship navigation services include caregiver assessment, case management, family support plan, navigation, advocacy, outreach and education, and support groups via a variety of supports and assistance to grandparents, other relatives, godparents, and nonrelative adults fictive kin with whom the children have a significant, positive relationship and emotional connection while being raised by them temporarily or permanently as follows: in the custody of dfs formal caregivers or not in the custody of but active with dfs in a living arrangement approved by dfs with a safety agreement in place voluntary caregivers. 2. as part of prevention, only information referral services and support group services provided to caregivers temporarily or permanently raising children not active with dfs. informal caregivers.
