This is a Request for Quotation (RFQ) for Underground Storage Tank (UST) inspection, testing, and maintenance services at Edward Hines, Jr. VA Hospital. The contract period includes a base year and four option years. A site visit is mandatory and scheduled for Tuesday, June 22nd, 2026, at 10:00 AM local time. Questions regarding the solicitation must be submitted via email by Wednesday, June 23rd, at noon local time. The contractor must be certified by the Office of the Illinois State Fire Marshall (OSFM) and adhere to all regulatory program inspection requirements. Services include inspection, maintenance, testing, and repair of USTs and ASTs, as well as preventative maintenance and fuel tank polishing. Invoices must be submitted electronically through Tungsten Network. Background checks and identification badges are required for all contractor personnel.
Quotes are due by the date specified in block 8 of the Standard Form **** or the most recent amendment.
Payments shall be made in arrears upon receipt of a properly prepared invoice, submitted electronically through the Veterans Affairs Financial Services Center (VAFSC) via Tungsten Network.
The contractor warrants and implies that the items delivered hereunder are merchantable and fit for use for the particular purpose described in this contract.
Firms submitting offers shall have been engaged in installing, repairing, testing, and maintaining UST systems for at least five consecutive years and must provide proof of Illinois Class A, B, and C UST certification. No subcontracting is permitted.
Failure to comply with facility requirements shall be considered a material breach of this contract and may be a violation of federal law. Failure to satisfactorily accomplish required inspections in a timely manner due to lack of parts/equipment may be justification to terminate the contract.
A site visit is strongly suggested and expected for all interested parties to inspect the campus. It is scheduled for Tuesday, June 22nd, 2026, at 10:00 AM local time.

Failure to comply with facility requirements, federal laws, or VA regulations may result in citation or charges answerable in the United States District Court.
