Proposals must be received by 3 p. m. Eastern Standard Time on 04/23/2025. The medical center's general terms and conditions and instructions to bidders apply. Contracts are governed by Kentucky law. Collusion is prohibited. Offerors must complete and sign an authentication of bid and statement of noncollusion and nonconflict of interest form. Offerors must comply with Kentucky Revised Statutes (KRS) **** and ****, and **** must be registered with the Kentucky Department of Revenue to collect and remit sales and use tax. Offerors must certify compliance with campaign finance laws (KRS ****). Offerors must submit final determinations of any violations of KRS chapters 136, 139, 141, 337, 338, 341, and 342 in the previous five years. Offerors must certify compliance with 41 CFR **** (nonsegregated facilities). Proposals must be submitted in two copies on electronic storage devices (USB) and seven printed original copies. Proposals received after the deadline will not be considered. There will be no public opening. Offerors may be required to make a presentation to the evaluation committee. Deviations from the RFP must be justified. Offerors should not submit standard terms and conditions as exceptions to the medical center's general terms and conditions. The medical center reserves the right to accept or reject any or all proposals. Proposals must address all criteria in section 4. 0. The contract term is three years, renewable for up to two additional one-year periods. The contract will be governed by Kentucky law. The medical center reserves the right to request modifications, reject all proposals, or cancel the RFP. Offerors may be asked to appear before the evaluation committee. The medical center reserves the right to add, delete, or change related items or services. The contractor shall procure all necessary permits and licenses and abide by all applicable laws, regulations, and ordinances. The contractor shall pay all applicable sales, use, personal property, and other taxes. The contractor shall procure and maintain minimum insurance coverages. The medical center reserves the right to reject any or all offers and to waive formalities and minor irregularities in the proposal received. A resident offeror of Kentucky shall be given a preference against a nonresident offeror. The contractor may be requested to provide a quarterly report of all products and/or services. The medical center recognizes an offeror's possible interest in preserving selected information and data included in the proposal.
