The project will fix these issues by installing a gabion weir, extending the existing block wall, and installing additional subdrains and ditches. The project site has previously been disturbed by coal mining operations, road construction, utility installation, residential development, landowner activities, and/or high velocity water flows associated with heavy rain events and flooding. These disturbances consist of significant upheaval, mixing, and removal of earthen material from deep excavation, grading, sub- surface drilling, fill material placement, and erosion. There should be no undisturbed earthen material to a depth of several feet at the areas slated for significant project- related construction activities. A cemetery up slope of the fill area will not be affected and the access road adjacent to the fill will be maintained as necessary to keep **** Three mine portals located behind the homes of Randy and Earl Johnson have led to oversaturation of their yards and a small slide. The portals will be corrected using non- wildlife accessible closures due to all portals being confirmed or suspected of being collapsed. AML will use drainage control methods such as subdrains and combination rock ditches to correct the issues. AM

New water service will be tapped from an existing Madison County Utilities 8- in water main. The point of connection will occur in the northern right of way of Fort Boonesborough Road ( KY 627) approximately 866 ft south of the Combes Ferry Road intersection with Fort Boonesborough Road. From this location, a new 8- in water main will be constructed and routed along the northern right of way of Fort Boonesborough Rd for approximately 4, 701 LF where it will cross Fort Boonesborough Rd at the entrance to Fort Boonesborough.
