Payment requires submission of pre and post-inspection reports with photos by 5:00 p. m. the following Thursday after work completion. all paperwork and photo documentation must be submitted by 5:00 p. m. the following thursday after work is completed in order for payment to be approved

The city intends to award to the responsive and responsible bidder whose bid is the lowest best value offer on the basis of cost per yard price. the lowest best value offer shall be defined as the lowest bid after adjusting for any mwbe price advantage

Bidders must demonstrate sufficient experience, equipment, and employees. Must submit list of owners/principals, statement of qualifications, equipment list, and W9. bidders must be able to demonstrate to the city that they have sufficient experience, equipment and employees to be able to perform the work specified herein

Failure to perform may result in a 5-day correction period, then the city may proceed against performance security or terminate the contract. failure of the contractor to correct such deficiencies may result in the city proceeding against the performance security

Bids may be rejected if unbalanced or inconsistent with current market prices. Bidders in arrears or default to the city will not be accepted. no proposal will be accepted from or contract awarded to any person, firm or corporation that is in arrears or is in default to the city

The authority shall pay for standard services for the not to exceed amount of in total annually to be paid on a monthly basis per site visit for each of the site locations per the contractors itemized bid in exhibit b.

The contract shall be awarded to the lowest experienced, responsible bidder whose bid is submitted in conformance with this notice.

The contractor represents and covenants that the contractor, its officers, employees, and agents have and shall possess the licenses, experience, knowledge, knowhow, qualifications, financial ability, and character necessary in order to fully perform the obligations of the contractor in accordance with the contract documents.

A civil penalty may also be assessed, not to exceed 25 of the total of wages, supplements, and interest due.

A mandatory site visit is needed for three locations that are more complicated in nature so that bidders can accurately quote for those systems.

The authority reserves the right to reject all bids if it shall be in the best interest of the authority to do so.
