The City of Elizabeth is seeking to prequalify medical service providers for fire, police, EMS, and civilian personnel. Services include pre-employment physicals, respiratory evaluations, and hepatitis inoculations. The contract term is September 1, 2026, through August 31, 2027, with an estimated value not to exceed $100,000. The submission deadline is June 30, 2026. Proposals will be evaluated based on qualifications, experience, knowledge, technical content, and cost reasonableness. This RFQ is not a bid and does not constitute a statutory bidding requirement.
Proposals must be delivered by 3:00 p. m. on Tuesday, June 30, 2026.
Acceptance of final payment by the contractor shall be considered a release in full of all claims against the City.
The City will select the most advantageous proposals based on relevance and extent of qualifications, experience, reputation, training, knowledge of the city, similar engagements, technical proposal content, reasonableness of cost, and other beneficial considerations.
Proposers must submit a technical proposal detailing the proposer's name, business location, education, qualifications, experience, and training of assigned personnel, and a statement that neither the firm nor assigned individuals are prohibited from professional practice.
Failure to submit required information, such as the business registration certificate or noncollusion affidavit, may result in automatic rejection of the bid or proposal.
The contract is open-end in an amount not to exceed $100,****.
