Luke Air Force Base anticipates a requirement for forklift loading bar extensions. The requirement is considered a total small business set aside. This is a request for quote (RFQ) for the rental of transportation services, including golf cars, utility vehicles, and a mini ambulance, for the US Secret Service in Sterling, VA. The anticipated period of performance is from July 1, 2025 to June 30, 2026, with a specific season from July 1, 2025 to October 31, 2025 and April 1, 2026 to June 30, 2026. This solicitation is for a supply contract for pharmaceutical products (ustekinumab) for the Department of Veterans Affairs (VA) and other federal agencies. Offerors must submit prices for the base year and four one-year options for all line items. Offers must include a 0. 5 cost recovery fee. Offerors must list an 11-digit NDC number for each drug. A letter of commitment from the manufacturer is required if the offeror is not the manufacturer. The contract will be effective on the date the contracting officer signs the **** and notifies the offeror. The implementation period is the timeframe beginning on the contract effective date and ending at the start of the ordering period. The ordering period begins at the expiration of the implementation period. The contract will be in effect for the duration of the implementation period plus one year ordering period with four one-year pre-priced option ordering periods. The contractor will receive payments from the prime vendor programs instead of directly from the government. Product registration with the FDA, First Databank, Medispan, and Cerner Multum is required within 5 calendar days of contract award. The government reserves the right to inspect records pertaining to sales under the contract. The contractor shall report all contract sales by calendar quarter and submit collected cost recovery fees within 30 calendar days following the completion of each reporting quarter or completion of the contract, whichever occurs first. The cost recovery fee payment is due to the VA fiscal division at the same time the sales report is due to the contracting officer. The contractor shall use only FDA-inspected manufacturing facilities for the duration of the contract. The successful offeror's label with its unique NDC shall be the only label on the product. All pharmaceutical products must include bar code labeling at the unit-of-use package level. The Drug Supply Chain Security Act (DSCSA) requirements apply. Subcontracting plan requirements apply for contracts valued over $750,000. The government will evaluate offers in accordance with the policies and procedures of FAR part 12, part 15, and part 25.
