DMVA is seeking optometry services for SWVC residents. The contract will be awarded to the lowest responsive quote. The term is one year with four one-year renewal options. The contractor must have at least 5 years of experience and a current Pennsylvania optometry license. Services include eye exams, screenings, referrals, and dispensing/repair of optical materials. Services are on an as-needed basis and billed against resident insurance. The contractor must be located within a 5-mile radius of SWVC. Compliance with federal, state, and local laws is required. Background checks and annual training are mandatory for contractor personnel. Invoices are to be submitted to DMVA for payment authorization.
Services shall begin as specified in the project statement or notice to proceed letter, whichever is the later.
The awarded contractor shall invoice each resident visit separately according to the bid price for an examination plus the itemized cost of materials. DMVA payment authorization is required prior to services. The contractor shall bill DMVA, and DMVA shall pay for services with DMVA payment authorization.
DMVA shall award the PO to the lowest responsive quote.
The awarded contractor shall provide proof of a minimum 5 years experience in optometry services and a current license to practice optometry with the Commonwealth of Pennsylvania.
If the awarded contractor creates a scenario where the contracting agency is found to be out of compliance with any requirements, the awarded contractor will be liable and responsible for any damages suffered by DMVA. Ongoing service deficiencies that are not resolved may result in termination of the contract.
Requests for submission of any necessary corrections or waiver appeals must be submitted by the awarded contractor within seventy-two 72 hours of request.
Contractor personnel shall not be employable by DMVA if they have been found guilty of abuse, neglect, exploitation, misappropriation of property or mistreatment by a court of law, or have had a finding entered into the Pennsylvania nurse aide registry or any state registry concerning abuse, neglect, exploitation, mistreatment of residents or misappropriation of resident property, or have disciplinary action in effect against their professional license.
