Bids will be accepted until August 7, 2026, at 11:00 AM. Immediate delivery of the vehicle is requested.

Invoices must include company name and address, invoice number, order number, quantity purchased and shipped, description of items shipped, and item price. No surcharges are allowed. All bid pricing submitted shall include all fees.

The bid must include the standard warranty for the vehicle. Warranty shall be included in pricing.

Bids will be awarded to the lowest responsible bidder, considering factors such as price, conformity to specifications, availability of repair parts, delivery times, ability to meet timeframes, and compatibility.

Bidders must submit a completed bid response form, a copy of a current Madison County business license (if required), Certificate of Compliance Economic Boycotts Act, Unauthorized Aliens Affidavit, Sworn Affidavit of Employer regarding Unauthorized Aliens, Sworn Affidavit of Subcontractor regarding Unauthorized Aliens, and E-Verify Memorandum of Understanding.

Bids may be rejected for failure to utilize bid forms, failure of E-Verify enrollment, lack of signature on notarized documents, incomplete bids, evidence of collusion, unauthorized alterations, or failure to submit required forms.

Quotes are due by 07/24/2026 at 12:00 PM EST.

Contractor shall receive payment for services rendered in accordance with the prompt payment act. Will be paid monthly, in arrears, upon receipt of a proper invoice for services performed during the previous month.

Award will be made to the quote that represents the best value to the government, considering price, technical, and past performance.

The contractor shall be an active member of the Maine Bar Association and in good standing. The contractor shall be regularly established in the business of guardianship, special proceedings regarding health care proxies.

Proposals must be received no later than 3:00 PM MST/MDT on September 02, 2026.

Payment is subject to availability of funds and will be tendered within 30 days after receipt of an approved invoice. Payments will be made through direct deposit only.

The contract will be awarded to the offeror whose proposal is most advantageous to the state, considering evaluation factors including technical and business specifications, and cost.

Offerors must be qualified and adequately trained, with licensed/certified practitioners meeting specific board requirements and being appropriately supervised. Experience in screening, assessment, individual therapy, and treatment plan development is required.

The procurement code imposes civil, misdemeanor, and felony criminal penalties for its violation. New Mexico criminal statutes impose felony penalties for bribes, gratuities, and kickbacks.

The protest deadline is September 30, 2026. The 15-calendar day protest period begins the day following the award of contracts.

Failure to complete and return the campaign contribution disclosure form, failure to submit a signed letter of transmittal, or non-compliance with mandatory specifications may result in disqualification.
