All questions must be submitted in writing via email no later than five (5) working days prior to the bid opening. All official communication will be in the form of an addendum. Acknowledge receipt of addenda on the bid form. Failure to comply with any requirement may result in bid rejection.

Offerors must complete and return all information designated in section E prior to the time specified in block 8 of SF **** pg 1. Responses must be received by 03/17/2025, 02:00 PM CST. Bidders must be registered in the LAgov system to submit an online bid. A W-9 form is required if the vendor is newly registered. All attachments must be printed and reviewed. The method of award is based on the lowest responsive, responsible bidder meeting specifications. The state reserves the right to reject individual line items. Bid prices must be typed, written in ink, or submitted online. Bid prices include delivery F. O. B. destination. Payment is due within 30 days of invoice or delivery. Bidders must specify brand and model number of the product. Bidders proposing an equivalent brand or model must submit descriptive information. All products must be new, never previously used, and the current model and/or packaging. The manufacturer's standard warranty applies. The contract may be extended for two additional 12-month periods at the same prices, terms, and conditions, but the total contract term cannot exceed 36 months. The state has the right to terminate the contract for various reasons, including misrepresentation, fraud, or failure to deliver within the specified time. All bid amounts must be submitted in US dollars.
