This is a combined synopsis solicitation for commercial items for contracted dental hygienist and dentist services to support the dental program at the Warm Springs Service Unit (WSSU) in Warm Springs, Oregon. The purpose is to provide supplemental professional dental coverage to ensure continuity of oral health services and patient access to dental care. Services include preventive, restorative, periodontal, pediatric, oral surgery support, patient education, and emergency treatment. The contract will be for one firm fixed-price purchase order with a one-year period of performance from the date of award. Offerors must be registered with the System for Award Management (SAM) and meet the definition of an Indian Economic Enterprise (IEE) throughout the offer, award, and performance periods. The award will be made on a best-value basis, considering price and non-price factors such as demonstrated prior experience and past performance. Quotes must be submitted electronically by the closing date. Questions regarding the solicitation must be submitted in writing via email no later than four business days prior to the closing date.
The period of performance shall be one year from the date of award.
Payment shall be made only for services actually rendered and accepted by the government. Payment shall comply with the prompt payment act and applicable federal regulations.
The government intends to award a contract to the respondent responsible whose quote represents the best value to the government, considering price and non-price factors.
The dentist contractor shall possess a DDS or DMD degree from an accredited dental school, current unrestricted dental license, DEA registration if applicable, minimum of two years of clinical experience or completion of a residency program, and current Basic Life Support (BLS) certification. The dental hygienist contractor shall possess graduation from an accredited dental hygiene program, current unrestricted dental hygienist license, current certifications required by state law for anesthesia or nitrous oxide administration if applicable, minimum of one year clinical experience preferred, and current BLS certification.
Any false or misleading information submitted by an enterprise when submitting an offer in consideration of an award set-aside under the authority of the Buy Indian Act is a violation of the law and punishable under 18 **** claims submitted as part of contract performance are subject to the penalties enumerated in 31 **** to **** and 18 U. S. C. 287.
Failure to maintain qualifications, licensure, credentialing, or privileges may result in immediate termination of the contract without cost to the government.
