Payment will be made in accordance with the pricing provided on Exhibit C. Invoices are submitted monthly and must describe work completed in detail.

Award will be made to the bidder who provides the best value to the state, considering bidder meeting or exceeding requirements, ability to meet performance delivery requirements, and price.

Minimum experience required is one year. Personnel must be fully trained and have at least one year of relevant experience. Contractor must be qualified to perform routine journeylevel habilitation or rehabilitation therapy work and be music therapist boardcertified (MT-BC).

Failure to comply with requirements may result in disqualification of the response, delayed payment, nonpayment, or termination of the contract.
