Invoices are to include the contract control number, dates, and a list of exact services performed, within thirty 30 days from the end of the monthly billing period.
The proposal that accrues the highest point total shall be recommended for award subject to the best interests of CIHA. The firm, fixed-price contract will be awarded to the contractor that submits the highest rated proposal which will be graded on the scoring criteria.
Proposals must include a statement as to the offeror's qualifications to perform the HVAC maintenance callout services described in the scope of services, and details on the qualifications of employees, technician resumes, years of experience, certifications, and equipment available to serve the contract.
An offeror who wishes to appeal a notice of intent to award must file a written protest within ten 10 calendar days following the date this notice is issued.
