A copy of current MLB Vendors License or a copy of the submitted application d. A copy of current insurance certificate e. A copy of Subcontractor/Material Supplier list f. A copy of valid State of Minnesota Contractors License g. A copy of Authorized Signature Sheet submitted with first bid submittal. Proposals not submitted in the specified mailing manner will be rejected.
