Delivery and installation of exam chairs and lights are required. The project is considered complete upon confirmation by VA stakeholders that all new exam chairs and lights are installed and operational, and all shipping materials, debris, and old chairs have been removed from the facility.

Payment will be made by the Department of Veterans Affairs, FMSVA2, 101 Financial Services Center. Payment is in accordance with the prompt payment act 31 **** and prompt payment regulations at 5 CFR part ****.

A 4-year extended warranty for parts and labor plan for the 626 barrier-free examination chair is included as item ****.

Award will be made to the responsible, technically acceptable offeror with the lowest total evaluated price. Technical acceptability will be evaluated on an acceptable/unacceptable basis.

Offerors must be certified SDVOSBs listed in the SBA certification database at the time of submission of their offer and at the time of award. They must also submit the full clause no-hyphenated version and no caveats of VAAR ****, with all fill-ins complete.

If VA determines that an SDVOSB/VOSB awarded a contract did not act in good faith, they may be subject to referral to the VA suspension and debarment committee, a fine under section 16(g)(1) of the Small Business Act, and prosecution for violating 18 ****.

Failure to submit the required certification for VAAR **** with the offeror's proposal will render the offeror ineligible for award. Willfully and intentionally misrepresenting a company's SDVOSB status can lead to debarment.
