Responses to this IFB must be received July 15, 2026, 10:30 am.

Payment will be made in accordance with chapter **** of the Texas Government Code, the Texas Prompt Payment Act. Invoices must be submitted monthly or as otherwise indicated on the purchase order.

Award will be made to the bidder who provides the best value to the state, considering bidder meeting or exceeding requirements, ability to meet performance delivery requirements, and price.

Bidders must have relevant experience of at least two years. Personnel must be fully trained with at least two years of relevant experience, hold a current license with the Texas Board of Dental Examiners, and be at least 18 years of age. Three verifiable references are required.

Failure to comply with sole point of contact requirements, submitting exceptions to the IFB, including assumptions in a response, not properly and timely notifying the sole point of contact of any ambiguity, conflict, discrepancy, or error, and failure to monitor the ESBD for addenda can lead to disqualification.

Bids must be received by the purchasing agency no later than August 26, 2026, at 10:00 AM. bids must be received by the purchasing agency on the solicitation due date no later than the solicitation due time as set forth in the solicitation.

The selected contractor will be paid upon satisfactory delivery/completion of work performed and submission of an invoice. he selected contractor will be reimbursed only for commoditiesservices performed and accepted by the commonwealth of pennsylvania.

Contractors must be board certified in endodontic services and possess an unrestricted license to practice endodontics in Pennsylvania. he selected contractor shall be board certified in the specialty of endodontic services and shall possess an unrestricted license to practice endodontics in the commonwealth of pennsylvania.

Failure to submit required items, such as cost submittal, board certification, license number, proof of insurance, and certification forms, may result in rejection of the contractor's submission. failure to submit these items may result in the rejection of the contractors submission.

Any future solicitation could only be awarded to a contractor who is registered in SAM. To receive award based on VOSB or SDVOSB status you must be registered in the SBA database.

The oral surgeon must possess a Doctor of Dental Surgery (DDS) or Doctor of Medicine (MD) degree, completion of oral maxillofacial surgery training from an accredited program, and be in good standing. Board certification is desired. The oral surgeon must possess a doctor of dental surgery dds or doctor of medicine md from a commission of dental accreditation coda or equivalent entity. Foreign trained surgeons must possess an equivalent degree. The oral surgeon must demonstrate completion of oral maxillofacial surgery training from an accredited surgical program. The oral surgeon must demonstrate good standing in all current facilities. Board certification in oral maxillofacial surgery is desired.

Responses to this IFB must be received July 8, 2026, 10:30 AM CT. The anticipated contract start date is the date of award.

Invoices must be submitted monthly or as otherwise indicated on the purchase order, detailing work completed. Payment will be made in accordance with the pricing on Exhibit C, after satisfactory performance is acknowledged.

Contractors must hold a DDS or DMD license, a Texas State Board of Dental Examiners (TSBDE) anesthesia permit with specific endorsements, and have a minimum of five years of relevant experience. Personnel must also be at least 18 years of age.

Failure to comply with communication requirements may result in disqualification. Noncompliance with contract terms may result in termination of the contract.
