This is a solicitation for dental lab services for the Birmingham VA Healthcare System. The contract period includes a base year and four option years. The contract period includes a base year from 07/01/2026 to 06/30/2027, with four one-year option periods. The solicitation states, Offerors have 30 days to complete all components of the solicitation.
Payment will be made by each participating medical facility either via electronic funds transfer or purchase order. Each facility will coordinate the method of payment with the awarded vendors.
The contractor shall guarantee complete satisfaction with workmanship and materials of the prosthesis. If the appliance is unacceptable, the item will be remade/adjusted at no charge. This warranty would not extend against normal wear and tear, accidental breakage or negligence.
The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government. The evaluation factors are technical samples, past performance, price, certifications, and locality, with all factors being of equal importance.
The contractor shall have all licenses, permits, and certifications as required by local and state authorities for the state of Alabama. The minimal certifications that shall be submitted include: CLIA accreditation 42 CFR 493 compliance, FDA QSR ISO ****, and NADL certification for the state of Alabama.
If the contractor fails to meet the standard of no more than 2 failed standard validated complaints per year, it may be cause for contract termination or corrective action.
All offerors will submit technical samples to the identified point of contact, at the predetermined location, date and time and outlined in the technical samples document. In accordance with the solicitation, offerors will pick up sample dental molds and fabricate dental casts as outlined in the aforementioned document.

Any omission or incompletion of the factors listed under FAR **** could lead to a rejected offer.
