Contra Costa Behavioral Health Services/Mental Health Plan is seeking a qualified community-based organization to provide Assertive Community Treatment (ACT) and Full Service Partnership (FSP) Intensive Case Management (ICM) for eligible Medical Transitional Age Youth (TAY) countywide. The contract period is anticipated from January 1, 2027, through June 30, 2029 (30 months). The total amount available is not to exceed $5. 2 million per fiscal year. Proposals are due August 28, 2026. Services must comply with federal, state, and local laws, DHCS requirements, and Medi-Cal and Medicare standards. Questions are due August 12, 2026, via BidNet Direct. The selected provider will create one ACT team and one FSP ICM team. ACT services are for individuals aged 18+ with SMI or co-occurring SMI/SUD, significant functional impairment, and high service needs. FSP ICM services are for individuals aged 18+ meeting similar criteria but with moderate to significant functional impairment. Both models emphasize multidisciplinary, team-based, field-based care. ACT teams must adhere to DHCS requirements for training, technical assistance, fidelity monitoring, and data collection. FSP ICM services are billable under Medi-Cal and do not have bundled rates or fidelity monitoring requirements. The selected bidder will enter into a standard Contra Costa County contract and must maintain required insurance coverage.

Proposals will be scored based on organizational structure and qualification (10 points), program narrative (60 points), program administration, quality oversight and compliance (20 points), and budget and budget narrative (10 points).
Qualified applicants are organizations that meet requirements including current SMHS Medi-Cal provider certification in good standing, or proof of a recently submitted and accepted application, and current Medicare enrollment/certification or proof of a recently submitted and accepted application.
