Contractor shall submit monthly/quarterly invoices, in the format approved by LCDSS, based on actual expenses, no later than the 20th of the month following the month in which services were provided, except for the months of May and June. For the months of May and June: Funding for this contract is appropriated on a fiscal year basis. LCDSS is not able to compensate contractor after the close of the fiscal year period.
Proposals will be evaluated based on costs of services and cost-effective methodology, reputation and experience, capability or potential to accomplish work, responsibility in the required time, performance on prior contracts, and the extent to which the proposal addresses the scope of work. Criteria for evaluation Proposals will include, but not be limited to, the following: Costs of services and cost-effective methodology in performing assignment.
Proposers must be qualified agencies with experience in providing services to diverse ethnic, linguistic, and cultural populations. They should demonstrate knowledge and skills to serve Lake County's diverse community and have a plan for ongoing staff training on diverse communities. Applying vendors and their staff must currently serve and be deeply embedded in the community in lake county.
Contractors must not be debarred or suspended from federal financial assistance programs, convicted of fraud, or have had public transactions terminated for cause or default within the last three years. The contractor certifies to the best of its knowledge and belief, that it and its subcontractors: 1. are not presently debarred or suspended from federal financial assistance programs. . .
