This document is an affirmation and disclosure form for contractors regarding the performance of offshore services, locating state data offshore, or purchasing from Russian institutions. The contractor must provide the locations where services will be performed and where data is located. Failure to provide this information may result in no award. The contractor must disclose any changes in location of services to the state immediately. The state has the right to terminate the contract if services are performed outside the United States, unless a waiver is obtained.

The government will award a contract to the most advantageous offeror, considering price and other factors. Evaluation factors include price (40%), technical capability (40%), and past performance (20%). Offerors must demonstrate compliance with federal cost principles, provide detailed pricing information (labor rates, post-production costs, pricing transparency), and submit a portfolio of relevant work samples. Past performance will be evaluated based on experience in wildland fire, emergency response, and natural resource environments, as well as the quality of past deliverables. Offerors must submit a list of relevant contracts and subcontracts completed in the last three years, and any similar projects currently in progress. If newly formed, offerors may submit relevant contracts performed by key personnel. Additional documentation may be required, including a copy of a current red card incident qualification card if claiming fireline or prescribed fire experience as a non-escorted contractor.
