The RFQ response due date is August 3, 2026, at 12:00 PM ET. this amendment is to respond to vendor questions to rfq **** and to extend the rfq response due date to 08/03/2026@12:00pm.

Failure to provide the required certification may result in the offer being deemed ineligible for evaluation and award. the government will not consider offers for award from offerors that do not provide the certification, and all such responses will be deemed ineligible for evaluation and award.

Delivery and acceptance are to be F. O. B. destination 60 days ARO at the Ralph H. Johnson VAMC.

The government will make payment in accordance with the prompt payment act 31 **** and prompt payment regulations at 5 CFR part ****.

The contractor warrants and implies that the items delivered under this contract are merchantable and fit for use for the particular purpose described in this contract.

The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered.

Offers shall include information necessary to evaluate the factors contained in the provision at **** or as described in the solicitation.

Failure to complete and sign the RFQ in its entirety shall not be considered for award.

Offers are due on or before 1:00 PM Eastern Standard Time EST, on 04 August 2026.

Monthly invoices shall be submitted via Wide Area Workflow (WAWF) in accordance with clause ****.

Contract employees must be licensed audiologists or physicians who have completed a residency in otolaryngology, licensed to practice in the state where the office is located. Audiometric technicians must be certified and supervised by a physician or audiologist.

Offerors that fail to furnish required representations or information or reject the terms and conditions of the solicitation may be excluded from consideration. No contractor who is chemically dependent or impaired, has been previously convicted of a felony, or is awaiting possible adverse administrative action by any certifying board shall be acceptable.

A separate invoice shall be submitted for each individual examined.

The contract will be awarded based upon the total low cost of the estimated services required over the length of the contract.

Physicians performing the examinations will be board certified by their respective organizations doctor of medicine or doctor of osteopathy and their status will meet all requirements of currency or will be board certified in internal medicine with documentation of continuing medical education CME in occupational medicine.

Proposals are due by 26 June 2026, 4:00 PM Eastern Time. Delivery is FOB destination.

The government will award a purchase order to the responsible quoter whose quote conforming to the solicitation will be most advantageous to the government on price and other factors considered.

This requirement is 100% set aside for small business, and only qualified vendors may submit quotes. Registration with the System for Award Management (SAM) is required.

Quotes must be valid for sixty (60) days. Registration with SAM must be active at the time of award.
