Lot 1: The Vendor will pay a commission to the Office of Vocational Rehabilitation ( OVR) based on gross sales of each machine. The vendor will enter commission for Each Machine - DO NOT ENTER % SIGN. Price will appear as a dollar amount but should be the percentage without the percent sign. The contractor will provide the Office of Vocational Rehabilitation with the name, address, phone number, etc. , of the distributor who will service each area no later than ten ( 10) working days after contract award. All snack machines will be new or like- new, current models, operating on 115V, equipped with tamper- proof cash accountability system( s). All machines must be equipped with bill acceptors. At a minimum, two ( 2) snack machines ( one standard, one refrigerated) will be required at each rest area vending building. The number required for weigh stations will be determined by the contractor. All machines will be subject to inspection by the Office of Vocational Rehabilitation and the Transportation Cabinet. All machines must meet applicable governmental mandates. The contractor will be provided a list of state personnel authorized to inspect machines. The number of machines may be increased or decreased only with approval of the Office of Vocational Rehabilitation and Transportation Cabinet. A list, showing number of machines, serial number of each, and identified by location, will be presented to the Office of Vocational Rehabilitation no later than ten ( 10) working days after installation is completed. Products to be sold will be nationally- known brands and will be offered in a variety of selections, i. e. , chips, candy, pastries, gum, cookies, crackers, nuts, etc. No out- of- date products may be offered for sale. Products must meet all standards of the National Food and Drug Administration and meet the approval of the Office of Vocational Rehabilitation. No health- related items will be sold through snack machines. Selling prices will be set to offer the best commission/rate of return to OVR. The following are the
