The contractor must be prepared to provide all required services starting January 1, 2025.

The contractor shall bill the residents dental insurance plans first, and then bill the AFRH only for amounts not covered by their insurance or for patients who do not have dental insurance. AFRH will be billed monthly.

The contractor shall provide a dentist, dental hygienist, and dental assistant with the following qualifications. All candidates shall have a minimum of two years professional experience. In addition, all licensed dental practitioners must submit to and pass the AFRH credentialing process.

At the CORs discretion, the contractor may incur a payment deduction for lack of compliance with the quality control plan, PWS or SDS.
