Required documents include a cover letter, forms for general bid and filed subbids (masonry and miscellaneous iron), unit price form, Somerville Living Wage form, quality requirements form, certificate of noncollusion, tax compliance, signature authority, reference form, bid deposit, prevailing wage statement, OSHA form, Somerville Vulnerable Road Users Ordinance form, Somerville Wage Theft Ordinance, Responsible Employer Ordinance (if applicable), acknowledgement of addenda (if applicable), signed W9, certificate of good standing (if applicable), insurance specifications, statement of management (if applicable), performance bond (100% of contract value), and payment bond (100% of contract value). The successful bidder will be required to submit a preliminary construction schedule within 10 days of the established date of award of the contract and a more detailed Gantt-type construction schedule within 15 days of the established award of the contract, showing the dates of substantial and final completion. The contract will be awarded within 90 days after the bid opening, extendable by up to 45 additional days by mutual agreement. The offeror's submission will remain in effect for 90 days from the response deadline or until it is formally withdrawn, a contract is executed, or this solicitation is canceled, whichever occurs first.
