The Ohio Department of Mental Health and Addiction Services (OHIOMHAS) is seeking proposals for the
sterilization of
medical instruments for the period of April 15, 2025, to June 30, 2027. Proposals must be submitted via the Ohiobuys website by 2:00 PM EST on the date indicated on the sourcing project. The RFP consists of 31 pages. All inquiries must be submitted through the Ohiobuys website. The proposal must include a technical proposal and a cost proposal, outlining how each deliverable will be completed within the specified timeframes. The contractor must demonstrate compliance with
sterilization and infection control guidelines per the Centers for Disease Control and Prevention (CDC) and other applicable requirements. The evaluation process may consist of up to four phases: initial review, evaluation committee review, request for more information, and negotiations. Evaluation criteria include years in business,
sterilization process, transport process,
sterilization turnaround time, and pricing. A minimum acceptable score is 300. Any contracts resulting from this RFP are subject to the terms and conditions of the personal services contract. The information contained in the RFP and in the proposal submitted by the selected contractor shall be considered part of the contract. Payments are contingent upon the availability of state and federal funds. The contractor must have an approved affirmative action plan recorded with the state of Ohio. The contractor must comply with all applicable state and federal laws regarding equal employment opportunity, including Ohio Revised Code section **** and all related executive orders. The contractor must keep all financial records in a manner consistent with generally accepted accounting principles. The contractor must keep separate business records for this project, including records of disbursements and obligations incurred. The contractor must maintain workers compensation insurance as required by Ohio law. The contractor warrants that it is not subject to an unresolved finding for recovery as defined in O. R. C. 9. 24. The contractor must comply with all relevant provisions of O. R. C. chapters 102 and ****, as interpreted by the courts of the state and the opinions of the Ohio ethics commission. The contractor is not boycotting any jurisdiction with whom the state of Ohio can enjoy free trade, including Israel, and will not do so during the contract period. The contractor shall not discuss or disclose any information or material obtained pursuant to its obligations under this agreement without the prior written consent of OHIOMHAS. The contractor agrees not to use advertising, news releases, sales promotions, or other publicity matters relating to any product or service furnished by contractor wherein OHIOMHAS's name is mentioned, or language used from which a connection with OHIOMHAS may be reasonably inferred, without the prior, written consent of OHIOMHAS. The contractor must comply with all applicable federal, state and local laws, rules, regulations and ordinances. No contract funds may be spent offshore. The contractor must complete the attached affirmation and disclosure forms.
