The warranty period for goods and/or services will be at least twelve 12 months from acceptance or delivery. Contractors' employees will have the necessary certifications and qualifications to perform services competently.
Quoters are required to have the applicable license as indicated in the service description table and must complete a quote certification form. Vendors must also ensure their employees and subcontractors are not debarred, suspended, or excluded from participating in federal health care programs.
Contractors who do not timely meet the program requirements will be removed from the program. Failure by a contractor to timely meet program requirements may result in termination of the contract.
The careoregon team will request contractors/vendors to complete a site visit in order to develop a quote based on contemplated services.
Communication in violation of restrictions may result in rejection of a quoter. Failure to comply with all requirements may result in quote rejection. Providing incorrect or incomplete information may be cause for rejection of the quote or contract termination.
