The contractor shall deliver portable cylinders, regulators, regulator keys and o2 carriers to the ann arbor vamc within one 1 business day of request to accommodate home oxygen beneficiaries who are visiting the va or being discharged and require a tank of oxygen to go home. The contractor shall deliver portable oxygen equipment to previously established and new patients being discharged to their home from nonva facilities and other facilities as requested by va staff per the needs of the veteran. equipment shall be delivered within four 4 hours of the request.

Invoices shall be submitted in arrears monthly. All payments for services rendered shall be paid for via **** purchase order number.

The government will award this procurement in accordance with FAR 12 and 15. see instruction to offerors insection e. 2 for specific award criteria.

Failure to deliver within the required time shall result in the va obtaining oxygen from another source. Any costs in excess of contract pricing for services provided shall be billed to the contractor or credited to next invoice at the discretion of the ordering facility.

Questions are due in writing no later than 15 days calendar days before the rfp close date in box 8.

The maximum value of this contract shall be no more than $45,000,**** over the life of the contract.

The bid notice states he contractor shall deliver all equipment required under this son within thirty 30 calendar days or less from the date of award.

The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered.

The solicitation is a total set-aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSB).

The estimated total contract price is the contractor was afforded an opportunity to provide a quote to continue the same services for a 12month bridge based on updated labor rates.

A valid contractor's license issued by the state of California is required.

Liquidated damages of $100 per day apply for each calendar day.

A pre-bid meeting is scheduled for October 7, 2025, at 10:00 AM PDT, held online via Zoom at https:fresno. zoomgov. comj1609588136. City staff will be present to answer questions and there may be an inspection tour of the job sites.

The VA requires the contractor to deliver within 30 days aro, no later than September 15th, 2025.

Invoices shall be submitted upon delivery and receipt of items.

The contractor shall provide a warranty per the specifications in this sow. The warranty shall meet or exceed standard manufacturer industry warranty terms. The government does not accept an extended warranty, which requires advance payment.
