Mohave is soliciting bids for kitchen exhaust hood system cleaning and repair services for its statewide membership of approximately 480 public agencies. Contracts will be for an initial one-year term and four potential one-year extensions. Bidders must have a current Arizona Registrar of Contractors license (C16 or CR16) or be in the process of obtaining one. Bidders submitting a cleaning-only bid must explain their exception to the licensing requirement. A list of current members is available on Mohave's website (www. ***. *. *). This solicitation is for encampment site waste removal and cleanup services in Orange County, California. The prime contractor must maintain specific licenses and trainings, including a Class A XBF general engineering contractor license, hazardous substance removal haz certification, hazardous materials certificates, trauma scene waste management practitioner certificate, waste tire hauler registration, hazardous waste transporter registration, confined space entry certification, and hazardous waste operations and emergency response (Hazwoper) certification. Subcontractors applying or spraying disinfectant must possess a pest control business license, qualified applicator license, and confined space entry certification. Proof of these requirements must be provided to Caltrans prior to applying or spraying disinfectant. Bidders must possess a valid registration with the Department of Industrial Relations (DIR) and a valid motor carrier permit from the California Department of Motor Vehicles (DMV). This is a procurement for new OEM medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment (replacement parts) for VA medical centers. No remanufactured or gray market items will be acceptable. Offerors must be OEMs, authorized dealers, authorized distributors, or authorized resellers, verified by an authorization letter or other documents from the OEM. All software licensing, warranty, and service associated with the contracts must be in accordance with the OEM terms and conditions. Inspect all screens, filters, door seals, gaskets, valves, valve kits, piping, unions, fittings, solenoids, etc. The services to the equipment will be performed Monday through Friday, 7:30 a. m. to 4:00 p. m. , local time, excluding federal holidays. The service contractor will provide factory trained and qualified service technicians and provide copies of training certificates. The service contract covers labor and travel charges for routine maintenance as well as six unscheduled maintenance visits. Unscheduled repairs will be made within **** hours. The contractor shall use reasonable care to avoid damaging existing buildings, equipment, and vegetation. The contractor shall comply with all applicable federal, state, and local safety and occupational health laws, rules, and regulations. The contractor shall utilize products and material from bio-based materials to the maximum extent possible. USDA APHIS PPQ is a secure facility. Contractors must be escorted at all times while on site. Contractors must provide photo ID and sign in/sign out when entering/leaving the facilities. The contractor shall submit invoices and supporting documentation to the COR and submitted via www. ***. *. * proper invoice must include specific information. The government reserves the right to reject an invoice that does not include the information above. The contractor shall comply with FAR clauses. The contractor shall include the substance of this clause in subcontracts. The contractor shall, without additional expense to the government, be responsible for obtaining any necessary licenses and permits, and for complying with any untied states federal, state, and municipal laws, codes, and regulations applicable to the performance of the work. The contractor shall also be responsible for all damages to persons or property that occur as a result of the contractor's fault or negligence. The contractor shall also be responsible for all materials delivered and work performed until completion and acceptance of the entire work.
