The contractor shall document all system actions and observations on a Field Service Report (FSR) for each site visited. Damaged components must be reported and evaluated, with minor damage repaired by the contractor. Severe damage not caused by the contractor or due to neglect is not covered unless separately funded. The contractor shall also inventory major CHAP system components and present a modernization plan. Applicable publications include the FAR and DFARS. Deliverables include a Quality Control Plan, designation of Contract Manager, on-site evaluation reports, safety device inspection reports, damage reports, modernization plans, and Field Service Reports.
The period of performance to complete CHAP preventative maintenance services shall be 120 days from the date of award.
The contractor must ensure that any personnel performing under a contract are fully trained, licensed, certified, and otherwise qualified to provide CHAP services. The government does not provide training to contractors.
