This is an Invitation to Bid for elevator maintenance and repair services for the Louisiana Department of Health, Eastern Louisiana Mental Health System (LDH, ELMHS) located at **** Highway 951, Jackson, LA 70748. The contract will be for a 12-month period beginning July 1, 2026, with the option to extend for two additional 12-month periods. Bidders must be registered in the LAGov system. A mandatory jobsite visit is required for all bidders. Mechanics must be licensed by the Louisiana Office of State Fire Marshal, and a copy of the license should be included with the bid. Bids are to be submitted electronically via the LAGov vendor portal. All attachments to the bid invitation must be reviewed and included in the submission. Payment terms are within 30 days after receipt of invoice or delivery. The state reserves the right to terminate the contract for cause or convenience. Bidders must certify compliance with various federal and state laws, including those related to civil rights, ADA accessibility, and prohibitions against discriminatory boycotts and discrimination against firearm industries. A W9 form is required for newly registered vendors.
The contract shall be effective for a 12 month period beginning July 1, 2026 and ending June 30, 2027. At the option of the State of Louisiana and acceptance by the contractor, the contract may be extended for two additional twelve 12 month periods at the same prices, terms and conditions. Total contract time may not exceed thirty-six 36 months.
Bids containing payment in advance or C. O. D requirements may be rejected. Payment is to be made within 30 days after receipt of properly executed invoice or delivery, whichever is later.
The manufacturers standard warranty will apply unless otherwise specified in the solicitation.
The bidder and its mechanics shall be licensed by the Louisiana Office of State Fire Marshal OSFM in accordance with LA R. S. 40: **** et seq. The bidder should include a copy of their license with their bid.
Default of contractor: Failure to deliver within the time specified or to provide the services as specified in the bid will constitute a default and may cause cancellation of the contract. Where the state has determined the contractor to be in default, the state reserves the right to purchase any or all products or services covered by the contract on the open market and to charge the contractor with cost in excess of the contract price.
A jobsite visit is mandatory for all bidders. Failure to comply will eliminate your bid from award consideration. Please refer to page 11 in attachment A special terms and conditions for completed details.
Failure to comply with the mandatory jobsite visit will eliminate your bid from award consideration. Any bidder who fails to include a copy of their license with their bid must provide a copy to the office of state procurement within 10 business days of a written request. Failure to do so shall result in the rejection of their bid.
