The New York State Department of Health is seeking a contractor to supply an FDA-approved intranasal naloxone (4 mg in 0. 1 ml solution with a 3-year shelf life), accompanying fentanyl test strip kits, and an informational card. The product must meet FDA requirements, be effective in reversing opioid overdoses, have peer-reviewed evidence of reduced precipitated withdrawal, and be practical for community bystander use. Fentanyl test strips' accuracy may vary. The contractor must ensure product quality before shipping, provide tracking numbers, and manually maintain shipment information. Deliveries will be made to over 1,041 programs and affiliate sites throughout New York State. Bidders must submit a completed and signed Attachment B bid form, including a firm, fixed price per unit for each year, covering all specified products, services, shipping (FOB destination), customs duties, and charges. The bid price per unit includes one naloxone unit (2 doses), one fentanyl test strip kit, and one informational card. Quantities are annual estimates. Prices are binding for 365 days from the proposal due date. Failure to comply with bid form requirements or submit the cost proposal form will result in disqualification.
