The contract is for Housing Case Management Services for families in Sun Prairie, Wisconsin, facing housing instability or homelessness. The contract term is from March 4, 2025, to December 31, 2026, with a possible one-year extension. The program will utilize a housing-first approach and must collaborate with other community-based organizations. The funding source is the Sun Prairie Affordable Housing Fund, with $130,000 available for 2026. The contractor must provide required forms, including a W9 form and a certificate of insurance.
The contractor shall complete the services under this agreement within the time for completion specified in the scope of services, including any amendments. The term of this contract shall be from tuesday, march 4, 2025 to thursday, december 31, 2026 with the option to extend for one 1 additional year period upon the same or more favorable terms and conditions, and under mutual agreement of both parties.
The city will pay the contractor for the completed and accepted services rendered under this contract on the basis and at the contract price set forth in section v of this contract. The city will pay the contractor for completed and approved extra services, if any, if such extra services are authorized according to the procedure established in this section.
Participants must be referred to the program through a Sun Prairie Area School District (SPASD) social worker or Colonial Club case management. Eligibility is limited to families with at least one child enrolled in the district or households age 60 and older, located in Sun Prairie at least 50% of the time.
In the event the contractor shall default in any of the covenants, agreements, commitments, or conditions herein contained, and any such default shall continue unresolved for a period of ten 10 days after written notice thereof to contractor, the city may, at its option and in addition to all other rights and remedies which it may have at law or in equity against contractor, including expressly the specific enforcement hereof, forthwith have the cumulative right to immediately terminate this contract and all rights of contractor under this contract.
For 2026 there is 130, 000 available from the citys ahf to support housing case managementservices and financial assistance.
