Contract period is 12 months with options to extend up to 48 additional months. Licensing and insurance requirements include workers compensation, general liability, and automobile insurance. Bidders must be qualified firms capable of providing the required services. The Statement of Contractor's Qualifications (Attachment C) and Profile of Firm Form (Attachment D) must be submitted by the apparent low bidder.
Failure to abide by directives, such as conducting work without prior written authorization, may release MPHA of any obligation to pay. Damage to MPHA property must be repaired timely. Unauthorized subcontracting may result in cancellation of the contract or forfeiture of funds.
Bids may be invalidated if additional marks, notations, or requirements are entered on the IFB documents. Failure to communicate with the buyer only may cause a bid to not be considered. Not meeting minimum requirements or being deemed not responsible can lead to disqualification.
