Contractors shall render invoices to the school district by the 15th day of each month for all work completed during the previous calendar month. Invoices shall be made on the standard form of the contractor and must include details of the work performed, hours, and materials used. Certified payroll must be maintained and submitted with all invoices for services rendered.
The contract will be awarded to the one bidder proposing the overall lowest cost: estimated total annual cost for part 1 plus estimated total annual cost for part 2 plus estimated total annual cost for part 3.
To be considered, proposals must include a certificate of incorporation in NY State and/or authority to do business in NY State, evidence of 5 years in business under the same name, a list of all awarded contracts in the last 5 years, a reference list including school districts where similar service is provided with contact information, a list of any persons owning 10 or more of the company, a completed proposal, a completed and notarized prevailing rate certification, NYSDOL certificate of registration, insurance certification, and a completed noncollusive bid proposal certification.
The contractor acknowledges that failure to obtain required insurance on behalf of the district constitutes a material breach of contract and subjects it to liability for damages, indemnification, and all other legal remedies available to the district. The district reserves the right to complete the contract or have it completed by others after delivery of a three-day notice to the contractor if the contractor does not man the job or causes undue delay of work. The cost to the district to complete the project will be deducted from monies otherwise due the contractor.
Bidders may visit the buildings at which the work is to be performed. Bidders are asked to contact the director of facilities, ***@***. *. *, in order to arrange for an appointment to survey the buildings.
Failure to submit the insurance certificate within five days of notification of the contract award will result in the immediate termination of the contract. The board of education will then award a new contract to the next lowest responsive and responsible bidder. If, in the opinion of the board of education, the contractor fails to correct inadequate service, the contract will be subject to termination.
