The deadline to submit proposals is June 12, 2026, by 4:00 PM HST.

Monthly payments will be made to successful applicants upon receipt and approval of a complete and accurate invoice, along with required documentation.

Proposals will be evaluated based on geographic service zones, applicant experience and capability, organization and staffing, administrative capacity, and service delivery approach, with a total possible score of 100 points.

Applicants must be public agencies or nonprofit private agencies with applicable licenses and certificates, and must demonstrate sufficient professional, qualified, experienced, and properly trained personnel.

The bid notice does not require sample submission.

A protest must be filed within five working days of the postmark of the notice of findings and decision.

A proposal may be automatically rejected for reasons such as failure to cooperate, inadequate accounting system, late proposals, inadequate response, not being responsive, or the applicant not being responsible.

The estimated funding for Kupuna Care Case Management Services may be up to $650,****.

The bid notice requires offerors to submit a standard qualifications questionnaire as part of their proposal, referenced in section 3. 1. 2 and attachment 04.

The bid due date is Thursday, May 28, 2026, not later than 2:30 PM.

The bidder shall be responsible for reading and examining the solicitation and/or bid documents, including any offer pages, scope of work, special provisions, exhibits, general terms and conditions, addenda if any, and any other sections or documents attached hereto, and all applicable requirements by law. Submission of a bid shall be deemed verification of such reading and examination.

Any request for approval to substitute any item or take exception to any specification, special provision or general condition, or clarification must be received through the OpenGov procurement system on or before Thursday, May 21, 2026 at 4:30 PM.
