The award will be based on the best overall proposal and best value, considering methodology, experience, expertise, knowledge translation, diversity, and cost-effectiveness.

Bidders must demonstrate significant experience serving Asian communities or collaborate with such an organization, possess in-depth knowledge of relevant methods, advertising strategies, qualitative/quantitative research, community input, and understanding of the gambling landscape in Massachusetts.

Failure to perform contractual obligations may result in corrective action, suspension, and/or termination. Vendor may be terminated or suspended from this contract for poor performance, failure to perform, nonresponsiveness, noncompliance, fraud or other cause.

Misrepresentation of SBPP or SDO certification status will result in disqualification. Failure to perform contractual obligations may also lead to disqualification.

The estimated value of the procurement is not to exceed $175,**** for the entirety of the project.

Proposals will be evaluated based on scoring criteria outlined in Appendix A, focusing on interest, alignment with mission, populations served, perinatal health impact, community needs, activities, reach, success measures, infrastructure development, and partnerships.

Proposers must be based in Oregon, be a culturally specific CBO, federally recognized tribe, tribal organization, urban Indian health program, or a nonprofit/for-profit entity currently providing perinatal services. Government organizations are not eligible.

OHA may reject a proposal if the proposer fails to comply with RFGP procedures, has delinquent debt to the state, has an unresolved contract dispute, fails to meet responsibility requirements, or makes inappropriate contact with state representatives.

Contra Costa Health will reimburse the contractor for actual costs monthly. Administrative expenses may not exceed **** of the award.

Proposals will be evaluated based on applicant capability, outreach and collaboration (30 points), geographic desirability for program participants (30 points), financial information (25 points), and service continuity (15 points).

Eligibility is limited to not-for-profit **** community-based organizations and hospitals or public agency service providers that serve in Contra Costa County. Agencies must demonstrate sufficient capacity and fiscal stability.

Contra Costa Health staff members may conduct a physical site visit to observe the facility and verify compliance with standards and requirements.

Appeals must be submitted in writing to the Family, Maternal, and Child Health Program Director within seven (7) business days of receiving written notification of the funding decision.

An agency with unresolved outstanding federal/state tax obligations is not eligible to apply for funding. Proposals not conforming to basic standards may be considered as not meeting the application deadline.

The award amount is up to $570,000 annually for fiscal years FY ****.
