Vendors must acknowledge and agree to the standard terms and conditions. They may need to provide a W9, a Conflict of Interest Questionnaire, and potentially a Certificate of Insurance if providing services. Criminal history checks and badging requirements are also outlined.
Failure to supply complete reference information may be grounds for disqualification. Late proposals will not be accepted. Vendors who have been awarded under previous solicitations are not eligible to respond.
