Contingency chemotherapy compounding services Chemotherapy compounding services, including provision of all chemotherapy agents, IV tubing, closed system drug transfer devices, and other necessary items. Aseptic compounding of hazardous chemotherapy agents based on patient- specific orders. Contractor supplies all drugs. Orders submitted via VA secure network or fax, confirmed by VA staff via phone or email. Compounded orders transported via courier, overnight mail service, or other means to ensure receipt prior to infusion date. Shipping costs included. Billing for medication utilized per individual patient. Compounding must be at an FDA 503a compliant facility, meeting state and federal laws. Labeling and beyond use dating based on USP 797, USP 800, and federal law. Contractor to provide certification reports for sterile compounding areas and hoods, with action plans for any failures.
