Milwaukee County is seeking a supplier for debt collection and tax refund intercept program processing. The winning offeror will act on behalf of the clerk of circuit court to collect debts, provide information and payment services to debtors, and provide trip processing. Services may be expanded in future years to include the Milwaukee County Sheriff's Office and Parks Department. The contract will be for a two-year term with two optional two-year renewals, for a total contract term of no more than six years. Documents required include insurance requirements, IT policies, proprietary information disclosure form, targeted business enterprise forms, EEOC compliance form, request for exceptions form, and sworn statement of offeror. Preliminary evaluation questionnaire and technical proposal questionnaires are required. Estimated annual commission payments and commission percentages are required for various payment methods.

This document defines the preventive maintenance and unscheduled maintenance for material handling equipment at commissary locations. The contractor is responsible for providing labor, supervision, equipment, tools, materials, parts, transportation, permits, and other necessary items. The government will not supervise contractor employees or control the method of performance. The contractor must coordinate activities with the contracting officer's representative (COR), manage the process for identifying equipment problems, and provide corrective action plans. The contractor must have personnel certified by the Automotive Service Excellence (ASE) and possess appropriate education, training, and experience. The contractor must comply with safety regulations, including OSHA, DOD, and DECA standards. The government will provide utilities. The contractor must report accidents to the COR or on-duty government manager. The contractor must maintain the cleanliness of the work area and dispose of hazardous materials according to regulations. The contractor must provide a service ticket for each PM and unscheduled repair or service to the COR, and the COR and contractor must sign each service ticket. The contractor must provide warranty documents for parts and materials. The contractor must maintain accurate time-keeping records. The contractor must notify the contracting officer if more than three unscheduled maintenance calls are made on the same piece of equipment per month. The contractor must comply with all applicable environmental laws, regulations, ordinances, and standards. Lubricating oils must be a minimum of 25% recovered materials, and engine coolants must be reclaimed.
