Payment for disposal costs incurred by the contractor at permitted final disposal facilities will be made at the cost incurred by the contractor as a pass-through cost without mark-up. Contractor must submit a copy of the invoice received by the disposal facility, an electronic copy tabulating all scale or load tickets, and proof of contractor payment.

Proposals will be evaluated based on experience (25 points), management team (20 points), resources (20 points), project approach (15 points), and fees (20 points). The award will be made to the highest scoring proposal.

The contractor must be duly licensed in accordance with the state of Oregon's statutory requirements. Proposers must be prepared to submit copies of all required licenses, certifications, and any other statutory requirements prior to signing a contract.

The contractor shall be responsible for taking corrective action in response to any notices of violations issued as a result of the contractor's or any subcontractors' actions or operations during the performance of this contract. Corrections for any such violations shall be at no additional cost to the government.

Providing incorrect or incomplete information in the proposal may be cause for rejection of the proposal or contract termination.

Delivery timelines are: FSS: 60 days after date of award; LRS: 30 days after date of award; MXG: 180 days after date of award.

Minimum 1-year warranty for all equipment. Warranty must cover all defects not caused by government misuse.

Offerors shall return a copy of this solicitation with all representations filled in. Offerors may elect to use their representations in sam. gov to fill this requirement.
