The State of Ohio is requesting bids for the replacement of sofas and chairs at Lake Hope State Park cabins. 90 sofas, 25 loveseats, and 120 chairs are required. The purchase order and origin number must be included on the invoice, along with the supplier's full name, ship-to address, unique invoice number, date of service, complete itemization of services, materials, goods, and labor furnished, line charges, and the remit-to address. The PO is valid only for goods and services received through June 30, 20xx. Any goods or services received after this date will require a new PO.
