Fluoride Treatment Government Bids & RFPs

The proposal due date is September 23, 2026, at 2:00 PM Pacific Standard Time.

Pricing is based on a per member per month (PMPM) rate. Options include a comprehensive full-risk solution or a third-party administrative (TPA) model.

Proposals will be evaluated based on experience with geriatric and medically frail populations, access and provider network, ability to provide services at home and at a PACE location, PACE model experience, and price.

Proposers must have at least five years of experience providing dental care to the elderly population, a minimum of two years experience with the PACE model of care, and demonstrated capability to provide utilization reports in the standard 837d format.

Protests relating to the content of the solicitation or an addendum must be filed within five business days after the date Caloptima releases the solicitation or addendum.

Proposals may be deemed nonresponsive if they are conditional, include material exceptions, or if proposers violate communication restrictions.

Bids are due no later than 12:00 noon, Tuesday, August 18, 2026.

The award, if made, will be to that responsible, responsive bidder whose bid, conforming to this solicitation, will be most advantageous to the authority, price and other factors considered.

The bidder recognizes the right of the macon water authority to reject a bid if the bidder failed to furnish any required submittals on the date required by the bidding documents, or if the bid is in any way incomplete or irregular.

Bids are due on Tuesday, August 11, 2026, on or before 2:00 PM local time. Deliveries of hydrofluorosilicic acid shall be made within seven days of the order.

The city will pay the fixed price bid per the quantities listed on the proposal page as long as the provided hydrofluorosilicic acid meets all applicable specifications. The city will not pay assay adjusted invoices. The pricing listed on the invoice shall reflect the contract pricing effective on the date that the order is placed.

Contracts will be awarded in accordance with section **** of the City of Canton codified ordinances using the standard of lowest and best bidder.

Bidders must comply with City of Canton income tax ordinances and may be required to submit articles of incorporation and a W9 tax form. Successful bidders will be required to complete and submit the bidder and contractor employment practices report and may need to submit an affirmative action plan.

The city of canton may impose a **** penalty to any bidder that withdraws their bid after the bid opening and prior to a contract award.

Failure to submit required documents, including Bid Form 1 and pricing/signature pages, may result in disqualification. Bids not received by the deadline will be disqualified. Failure to acknowledge all official addenda may also result in disqualification.

The contract time for the project is 365 days.

Prospective bidders may examine the bidding documents at Reynolds City Hall, 3 East William Wainwright Street, Reynolds, GA ****, or at the office of the engineer at Watkins Associates, LLC, Monday through Friday, between the hours of 8:00 a. m. and 5:00 p. m.

No bid may be withdrawn after the closing time for receipt of bids for a period of sixty 60 days.

Responses to this IFB must be received by July 22, 2026, 10:30 AM CT.

Payment will be made in accordance with chapter **** of the Texas Government Code, the Texas Prompt Payment Act. Invoices must include the contract or purchase order number and detail the work completed.

The award will be made to the bidder who provides the best value to the state of Texas, considering meeting or exceeding requirements, ability to meet performance delivery, and price.

Bidders must have at least five years of relevant experience. Personnel assigned to perform services must be fully trained and have at least five years of relevant experience. The contractor must have a DDS degree.

Failure to comply with sole point of contact requirements, including discussions about cost information, may result in disqualification. Including assumptions in a response could also lead to disqualification.

The historical spend for the same or similar services was $45,**** for the period of one year.
