This is a Request for Proposals (RFP) from Chemung County for nursing facility staffing services at the Chemung County Nursing Facility. Proposals must be submitted electronically via the Empire State Bid System website (www. ***. *. *) by 2:00 PM on June 18, 2026. Vendors are required to register on the website to obtain bid documents and receive addenda. The contract will be for an initial one-year term, with the possibility of two additional one-year extensions. The contractor will provide supplemental nursing personnel, including nurse practitioners, physician assistants, registered nurses, licensed practical nurses, and certified nurse aides, based on the facility's needs. The contractor must maintain personnel files with required documentation, including licenses, certifications, and health records. Invoices must be submitted weekly with supporting timesheets. The county will conduct criminal history background checks and the contractor will reimburse the county for these costs. A minimum of four hours' notice is required for cancellation of assignments by the contractor. The county reserves the right to reject any or all proposals. The contractor shall submit weekly invoices to Chemung County Nursing Facility each week for all services rendered over the preceding seven days. Each invoice must include necessary information pursuant to the pricing schedule set forth in schedule b, fees and expenses, and other information, as follows: invoice date and invoice number, week ending and period covered, dates assigned nursing professionals worked, nursing professionals name, title and shift worked, total hours worked and hourly billing rate and total for the shift. Each invoice shall be accompanied by copies of pertinent timesheets, verified and approved by the nursing professional and by the nursing services office. In the event of a discrepancy between Chemung County Nursing Facility timesheet and the vendors timesheet, Chemung County Nursing Facility timesheet will be the document that governs payment for services rendered.
The county will take into consideration the level of effort, technical quality, and responsiveness to the scope of contract services. Other factors which the selection committee will take into account in recommending a contractor shall include but not be limited to: specific knowledge and expertise in providing healthcare staffing services, preferably within longterm care or skilled nursing facility environments. 25 overall qualifications and experience. 25 key personnel assigned to the project. 15 price proposal. 35
Proposers shall provide a brief history of your firm, current workforce size, location, and other relevant background information documenting the minimum number of qualifying years of experience. Identification of account executive involved in this contract and their relevant experience. List and description of other nursing professional contracts. Documentation of financial condition including most recent financial statement. Reference contact information for at least five current clients. Any other information which will demonstrate and document the qualifications required.
Cancellation of an assignment by the contractor or assigned nursing professional with less than four 4 hours notice may result in a credit equal to four 4 hours at the scheduled rate being applied against contractors charges. Repeated instances of nocallnoshow, late callins, early departures, or failure to provide adequate coverage may be considered a performance deficiency. The contractor shall monitor and address attendance and reliability issues of assigned nursing professionals. Failure to do so may result in corrective action by the county, up to and including removal of assigned staff or termination of the agreement.
Should a discrepancy be found in, or omissions from the specifications, requirements for contract, or bid proposal form, or should the bidder be in doubt as to their meaning, they shall at once, no later than seven days prior to submission of proposal, notify the purchasing director in writing who will send written addenda to all bidders where necessary.
Any proposal received after 2:00 p. m. on the due date will not be considered. Incomplete submissions may be deemed nonresponsive. Repeated instances of nocallnoshow, late callins, early departures, or failure to provide adequate coverage may be considered a performance deficiency and may result in corrective action by the county, up to and including removal of assigned staff or termination of the agreement.
