Numerous attachments are required, including bid addenda acknowledgement, bid schedule pricing form, MWBE goal participation information packet, certification regarding lobbying, MWBE utilization plan commitment form, statement of bidder qualifications, references, contractor profile, prevailing wage rate instructions, bid bond confirmation and bid check authorization form, storm
debris removal map, performance bond, CPIW based price adjustment calculations, payment bond, certification of compliance with federal standards requirements, minimum insurance requirements, workers compensation insurance coverage, required contract provisions, standards specifications, and federal IFB general conditions and specifications. Bidders must complete and submit Form ****, Certificate of Interested Parties, electronically with the Texas Ethics Commission prior to the proposal deadline. Bidders must also submit any applicable licensing and certifications, and documentation that the bidder or its subcontractors is HubCertified by the Texas Comptroller of Public Accounts or the local MWBE office. A conflict of interest questionnaire must be filed with Harris County. Bidders must verify that their company and principal are not debarred through the System for Award Management (SAM). A sample insurance certificate must be provided. The bid schedule pricing form (Excel) is also required.
