The deadline to submit proposals is June 12, 2026, by 4:00 PM HST.

Monthly payments will be made to successful applicants upon receipt and approval of a complete and accurate invoice, along with required documentation.

Proposals will be evaluated based on geographic service zones, applicant experience and capability, organization and staffing, administrative capacity, and service delivery approach, with a total possible score of 100 points.

Applicants must be public agencies or nonprofit private agencies with applicable licenses and certificates, and must demonstrate sufficient professional, qualified, experienced, and properly trained personnel.

The bid notice does not require sample submission.

A protest must be filed within five working days of the postmark of the notice of findings and decision.

A proposal may be automatically rejected for reasons such as failure to cooperate, inadequate accounting system, late proposals, inadequate response, not being responsive, or the applicant not being responsible.

The estimated funding for Kupuna Care Case Management Services may be up to $650,****.

The opening datetime for proposals is May 06, 2026, at 3:00 p. m. Proposals must be submitted in a sealed envelope to the Franklin County Manager's Office.

The proposals will be reviewed based on the following criteria: qualifications (25 points), experience (25 points), program description (20 points), and cost proposal (30 points).

Providers must be a valid New York State licensed Home Care Services Agency (LHCsa) in Franklin County.

The bid notice states that interested vendors must send their information and/or capability statement and any questions by email no later than 10:00 AM Pacific Standard Time, April 27, 2026.

Capability statements and authorized distributor letter if applicable are also requested.
