This is a Request for Quote (RFQ) for onsite phlebotomy services to support the VA Caribbean Healthcare System. The contract will be an Indefinite Delivery Indefinite Quantity (IDIQ) with one base year and four option years. All questions concerning the RFQ must be submitted via email to the contracting officer no later than April 23, 2026, at 3:00 PM EST. Offerors must be registered in SAM and verified as a small business. The evaluation will be based on Technical Capability (Qualifications/Experience of Candidates and Staffing Plan) and Past Performance, with Technical Capability being significantly more important than price. Price will be evaluated by adding the total price for all options to the total price for the basic requirement, and one-half of the offerors final option year price to establish the total evaluated price.
The period of performance for the base year is from 07/01/2026 to 06/30/2027, with four one-year option periods. The contract maximum is $3,000,**** total inclusive of the base period and all options. No further task orders will be issued if the maximum aggregate amount is reached prior to the end date of the final period of performance.
The bid notice states that payment will be made by the Department of Veterans Affairs Financial Services Center. Invoices shall be submitted monthly. The contractor shall accept payment for services rendered under this task order as payment in full, and VA beneficiaries shall not be charged for services rendered by the contractor.

Phlebotomists assigned by the contractor shall have six months of specialized experience performing phlebotomy procedures, be fully trained and competent, and possess the ability to speak, understand, read, and write English and Spanish fluently. They must also meet all VA requirements for training, including BLS, CPR, HIPAA, and security awareness. Specific tests such as PPD, rubella, varicella, Tdap, and influenza vaccinations are also required.
The bid notice mentions that failure to comply with certain requirements may result in termination of the task order for breach.
