The contractor must maintain all insurance coverage with the minimum amounts described throughout the life of the contract. The contractor must provide a request for a price adjustment in writing to the hhs agency contract manager. The contractor must provide supporting documentation to justify any price adjustment. The contractor must notify hhsc in writing at least ten calendar days prior to the effective date of the discount. The contractor must submit a form ****, certificate of interested parties, to the state agency at the time the business entity is notified it is a potential awardee or at the time the business entity submits the signed contract to the agency.

The contractor must provide key personnel resumes and proof of relevant experience within one week of contract award. The contractor must submit a phase-in/phase-out plan within 10 business days of contract award. The contractor must comply with all security requirements, including background checks, training (anti-terrorism, IWATCH, operational security, threat awareness), and key control procedures. The contractor must submit an environmental plan within 30 business days of contract award. The contractor must report all contractor labor hours through SAM (System for Award Management) by October 31 of each calendar year. The contractor must provide all necessary vehicles and equipment, which must be clearly identified. The contractor must maintain the free flow of vehicle and pedestrian traffic while conducting grounds maintenance tasks. Any damage to government real property must be restored to the original state at no additional cost to the government.
