Failure to defend the position in district court within seven calendar days of notice regarding confidential information may result in the release of requested records. Should respondent fail to bring an action toenforce respondents rights within seven 7 calendar days of notice, then the requested records will bereleased.

Contractor must be pre-qualified as listed by the pre-qualification review board under the provisions of ordinance no. 26, 614 approved by city council on december 8, 2020. The pre-qualification areas acceptable for this project are as follows: demolition contractor "A".

The bid will not be considered unless this form has been fully completed and signed and certified by the bidder.

The requirements of the contractor& rsquo; s pre- qualification pursuant to ordinance no. 26, 614 approved by city council on december 8, 2020, are waived ( for the purpose of bidding only) for this project. all other requirements of the pre- qualification ordinance ( working in a public right- of- way) are not waived. in lieu of pre- qualification, bidder is required to submit other evidence of knowledge and experience relating to bidder& rsquo; s abili - attach completed form to bid
